SUMMARY:Aspen Lung Conference
DESCRIPTION:The 63rd Annual Thomas L. Petty Aspen Lung Conference\nWill convene at the Gant Conference Center in Aspen\, Colorado with a virtual/in-person hybrid format. The theme of the conference will be “ARDS in the 21st Century: New Insights into Clinical and Mechanistic Heterogeneity.” \nThe Acute Respiratory Distress Syndrome (ARDS) represents a common syndrome arising from numerous\, heterogeneous\, and often-independent disease processes.  Accordingly\, a single ARDS study drug\, if uniformly applied to all patients regardless of underlying disease pathophysiology\, would likely fail to consistently improve patient outcomes. For clinical studies to overcome this obstacle\, a new appreciation of ARDS mechanistic and phenotypic heterogeneity is required\, enabling future precision medicine approaches to the recognition and treatment of lung injury. \nThe 2021 Thomas L. Petty Aspen Lung Conference will integrate basic\, translational\, and clinical approaches to address the impact of ARDS heterogeneity\, with a focus on (1) understanding the presence and therapeutic significance of ARDS mechanistic and phenotypic subtypes\, (2) exploring multi-cellular and multi-systemic mechanisms responsible for this heterogeneity\, and (3) determining how to best account for disease heterogeneity during clinical trial design and outcome assessment. Heterogeneity within COVID-19 ARDS\, as well as between COVID-19 and non-COVID-19 ARDS\, will also be discussed. \nThis conference will provide an international forum bringing together leading basic\, translational\, and clinical ARDS researchers while welcoming trainees in pulmonology and critical care medicine\, with the goal of identifying shared interests that will lead to more productive research and more effective personalized therapies. SUMMARY:Rare Genetic Causes of Bronchiectasis: Paving the Way for Interventional Trials
DESCRIPTION:This conference will bring together clinical and translational investigators in rare genetic causes of bronchiectasis\, including primary ciliary dyskinesia (PCD)\, primary immunodeficiencies (PID)\, alpha-1 anti-trypsin disease (AATD) and cystic fibrosis (CF)\, to discuss preparing for collaborative multicenter clinical trials. In this day and a half-long conference presenters will address critical knowledge gaps in disease pathogenesis\, clinical trial preparedness and overcoming barriers to successful trials in these rare disease patient populations. Speakers will provide state-of-the-art updates on the diagnosis and major sources of morbidity in these conditions\, as well as commonalities and differences in bronchiectasis pathogenesis\, with an emphasis on potential therapeutic targets. Lessons learned from previous clinical trials\, particularly in CF\, will be presented\, in order to capitalize on previous experience as we plan future clinical trials. The strengths and weaknesses of potential clinical trial endpoints will be discussed. Innovative study designs for rare diseases and partnering with stakeholders will be highlighted. Junior investigators will be invited to participate in special sessions regarding career development and funding opportunities. DESCRIPTION:Severe asthma is defined as uncontrolled asthma despite optimal therapy and appropriate management of comorbidities; it affects up to 10% of adults with asthma\nIt is now broadly classified into 2 distinct endotypes based on the level of Th2-mediated inflammation:\n\nT2-high asthma is driven first by epithelial alarmins\, including IL-25\, IL-33\, and TLSP\, then by Th2-cytokines\, namely IL-4\, IL-5\, and IL-13\, and may include either a robust eosinophil or allergic response\nIn contrast\, T2-low asthma is driven by Th1 and Th17 cells\, IL-6\, IL-8 and IL-17\, and a neutrophilic or paucigranulocytic response\n\n\nTargets for biologic therapy for T2-high asthma include IgE\, the IL-5 signaling pathway\, and the IL-4R\n\nOmalizumab targets circulating IgE\nMepolizumab and reslizumab target circulating IL-5\nBenralizumab targets the α subunit of the IL-5R\nDupilumab targets the α subunit of the IL-4R\, which is involved in IL-4 and IL-13 signaling\n\n\nClinically relevant biomarkers for selecting a biologic agent include serum IgE level (omalizumab)\, blood eosinophil level (all 5 agents)\, and fraction of exhaled nitric oxide or FeNO (omalizumab and dupilumab)\nPatients with OCS-dependent asthma are also considered to have T2-high asthma; dupilumab is approved for this indication\, and other biologics have demonstrated efficacy in such patients as well\nCurrently no targeted therapies are approved for T2-low asthma; tiotropium\, macrolide antibiotics\, bronchial thermoplasty\, and/or low-dose OCS may be considered
