SUMMARY:Cleveland Clinic Advanced Diagnostic Bronchoscopy Workshop CME
DESCRIPTION:Expert presenters will discuss the most recent diagnostic tests and procedures along with how they can be used to prevent delays in diagnosis and to eliminate more invasive tests and their accompanying complications. The faculty will focus on improving knowledge and skills with endobronchial ultrasound transbronchial needle aspiration (EBUS-TBNA)\, guided bronchoscopy with radial probe ultrasound\, electromagnetic navigation\, and robotic bronchoscopy. In addition\, how to use convex probe EBUS for the concurrent diagnosis and staging of suspected lung cancer to reduce high procedure-related complications and a larger number of invasive procedures will be discussed. \nImportantly\, the organizers provide hands-on sessions during which participants can practice use of EBUS-TBNA\, electromagnetic navigation\, robotic bronchoscopy\, real-time CABT (C-arm based tomography) navigation\, and insertion of indwelling pleural catheter\, all under the guidance of experts in these fields. \n15.75 (7.75 for lectures only as an internet live activity) AMA PRA Category 1 Credits™ \n15.75 (7.75 for lectures only) ABIM MOC points \n15.75 (7.75 for lectures only) ANCC contact hours \nVenue: Loews Portofino Bay Hotel

SUMMARY:Strategies to Improve Diagnosis and Treatment of Nontuberculous Mycobacterial Lung Disease
DESCRIPTION:More than 200 species of nontuberculous mycobacteria exist that produce both pulmonary and extrapulmonary disease\nAccording to the National Managed Care Claims Database (Optum)\, the prevalence of NTM-LD is increasing\, and approximately 27 million individuals have the disease\nRisk factors for NTM-LD include underlying conditions (eg\, CF/CFTR anomalies\, AAT anomalies\, COPD\, pneumoconiosis\, collagen vascular disease\, bronchiectasis\, TNF alpha antagonists\, alveolar proteinosis\, chronic aspiration)\, significant environmental exposures (eg\, aerosolized soil and water\, endemic areas)\, and innate host issues (eg\, postmenopausal women\, thin women with distinct body habitus\, predisposition to hypersensitivity)\nDifferent clinical phenotypes for nodular/bronchiectatic and fibrocavitary NTM-LD disease include age\, sex\, body type\, and smoking status\nMedian time from symptom onset to diagnosis is 10 months with a range of 1 month to 6 years\nAccording to the 2020 ATS/ERS/ESCMID/IDSA clinical practice guidelines for NTM-LD\, a 3-drug\, macrolide-containing regimen is recommended and can be administered intermittently (noncavitary) or daily (cavitary); parenteral amikacin (including amikacin liposome inhalation solution) or streptomycin is suggested in patients with cavitary\, extensive bronchiectatic\, or macrolide-resistant disease\nIn patients who meet the diagnostic criteria for NTM-LD\, treatment should be started (ie\, rather than watchful waiting)\, especially in the context of positive acid-fast bacilli sputum smears and/or cavitary lung disease\nFactors related to disease progression include bacterial load (ie\, smear positive) and radiographic extent of disease; other predictors are older age\, low BMI (< 18.5)\, comorbidities\, low albumin\, anemia\, and elevated inflammatory indices\nPatients face many challenges with NTM-LD and experience real fears and struggles and losses in quality of life; it is important to share goals\, set expectations\, and provide education on adverse event management

SUMMARY:Respiratory viruses: New Frontiers 2022
DESCRIPTION:This Keystone Symposia conference will focus on novel insights into respiratory virus biology that may ultimately lead to more effective prevention and treatment strategies. In many cases\, these novel insights are made possible by new tools that are now available to study pathogens whose infectious cycle is largely restricted to the airways. New information about virus-host interactions will be discussed\, as well as the interplay among diverse microorganisms present in the host airways. The long-term ramifications of respiratory virus infections beyond acute disease will be explored along with both recent successes and failures in prevention and treatment. This conference is a joint symposium with Viral Immunity: Basic Mechanisms and Therapeutic Applications and will have a few combined sessions.

SUMMARY:Deconstructing Difficult-to-Treat Asthma in the Age of Targeted Biologic Therapies
DESCRIPTION:Severe asthma is defined as uncontrolled asthma despite optimal therapy and appropriate management of comorbidities; it affects up to 10% of adults with asthma\nIt is now broadly classified into 2 distinct endotypes based on the level of Th2-mediated inflammation:\n\nT2-high asthma is driven first by epithelial alarmins\, including IL-25\, IL-33\, and TLSP\, then by Th2-cytokines\, namely IL-4\, IL-5\, and IL-13\, and may include either a robust eosinophil or allergic response\nIn contrast\, T2-low asthma is driven by Th1 and Th17 cells\, IL-6\, IL-8 and IL-17\, and a neutrophilic or paucigranulocytic response\n\n\nTargets for biologic therapy for T2-high asthma include IgE\, the IL-5 signaling pathway\, and the IL-4R\n\nOmalizumab targets circulating IgE\nMepolizumab and reslizumab target circulating IL-5\nBenralizumab targets the α subunit of the IL-5R\nDupilumab targets the α subunit of the IL-4R\, which is involved in IL-4 and IL-13 signaling\n\n\nClinically relevant biomarkers for selecting a biologic agent include serum IgE level (omalizumab)\, blood eosinophil level (all 5 agents)\, and fraction of exhaled nitric oxide or FeNO (omalizumab and dupilumab)\nPatients with OCS-dependent asthma are also considered to have T2-high asthma; dupilumab is approved for this indication\, and other biologics have demonstrated efficacy in such patients as well\nCurrently no targeted therapies are approved for T2-low asthma; tiotropium\, macrolide antibiotics\, bronchial thermoplasty\, and/or low-dose OCS may be considered

SUMMARY:Rare Genetic Causes of Bronchiectasis: Paving the Way for Interventional Trials
DESCRIPTION:This conference will bring together clinical and translational investigators in rare genetic causes of bronchiectasis\, including primary ciliary dyskinesia (PCD)\, primary immunodeficiencies (PID)\, alpha-1 anti-trypsin disease (AATD) and cystic fibrosis (CF)\, to discuss preparing for collaborative multicenter clinical trials. In this day and a half-long conference presenters will address critical knowledge gaps in disease pathogenesis\, clinical trial preparedness and overcoming barriers to successful trials in these rare disease patient populations. Speakers will provide state-of-the-art updates on the diagnosis and major sources of morbidity in these conditions\, as well as commonalities and differences in bronchiectasis pathogenesis\, with an emphasis on potential therapeutic targets. Lessons learned from previous clinical trials\, particularly in CF\, will be presented\, in order to capitalize on previous experience as we plan future clinical trials. The strengths and weaknesses of potential clinical trial endpoints will be discussed. Innovative study designs for rare diseases and partnering with stakeholders will be highlighted. Junior investigators will be invited to participate in special sessions regarding career development and funding opportunities. Patients and families will have the opportunity to participate in “Meet the Expert” sessions. \nAlso\, on the agenda: \n\nMucous Rheology & Mucus Clearance\nAirway Inflammation and Oxidative Stress\nMicrobiome and Respiratory Infection\nDysregulated Immunity (Innate and Adaptive)\nLung function: Spirometry & Lung Clearance Index\n\n8.75 AMA PRA Category 1 Credits™ – maximum \nVenue: Children’s Hospital Colorado

SUMMARY:Aspen Lung Conference
DESCRIPTION:The 63rd Annual Thomas L. Petty Aspen Lung Conference\nWill convene at the Gant Conference Center in Aspen\, Colorado with a virtual/in-person hybrid format. The theme of the conference will be “ARDS in the 21st Century: New Insights into Clinical and Mechanistic Heterogeneity.” \nThe Acute Respiratory Distress Syndrome (ARDS) represents a common syndrome arising from numerous\, heterogeneous\, and often-independent disease processes.  Accordingly\, a single ARDS study drug\, if uniformly applied to all patients regardless of underlying disease pathophysiology\, would likely fail to consistently improve patient outcomes. For clinical studies to overcome this obstacle\, a new appreciation of ARDS mechanistic and phenotypic heterogeneity is required\, enabling future precision medicine approaches to the recognition and treatment of lung injury. \nThe 2021 Thomas L. Petty Aspen Lung Conference will integrate basic\, translational\, and clinical approaches to address the impact of ARDS heterogeneity\, with a focus on (1) understanding the presence and therapeutic significance of ARDS mechanistic and phenotypic subtypes\, (2) exploring multi-cellular and multi-systemic mechanisms responsible for this heterogeneity\, and (3) determining how to best account for disease heterogeneity during clinical trial design and outcome assessment. Heterogeneity within COVID-19 ARDS\, as well as between COVID-19 and non-COVID-19 ARDS\, will also be discussed. \nThis conference will provide an international forum bringing together leading basic\, translational\, and clinical ARDS researchers while welcoming trainees in pulmonology and critical care medicine\, with the goal of identifying shared interests that will lead to more productive research and more effective personalized therapies. Finally\, the varied scientific themes and therapeutic strategies emerging during the conference will be reconciled in a concluding Conference Summary presented by Dr. Thomas Martin (University of Washington).
